	  <title><![CDATA[Update from OSCON: The Open Source Movement Tackles Health Care RT @ahier]]></title>
	  <description><![CDATA[<p>
	The O&#39;Reilly Open Source Conference (<a href="http://www.oscon.com/oscon2010" rel="nofollow">OSCON</a>) was held this past week in Portland, Oregon. The annual conference is an opportunity to share ideas, skills, and best practices in the development and implementation of <a href="http://www.justmeans.com/What-is-open-source-why-should-you-care/15675.html">open source</a> technology.</p>
<p>	&quot;Open source,&quot; as we&#39;ve <a href="http://www.justmeans.com/What-is-open-source-why-should-you-care/15675.html">written before</a>, involves &quot;opening&quot; up the source (in the case of technology, this is typically the source code) so that others beyond the original creators can develop, expand, and modify the code. Unlike proprietary systems in which you are forbidden to &quot;open the hood&quot; to tinker with the moving parts, open source allows anyone to download the code and then alter it without restriction or fear of punishment.</p>
<p>	Many people argue that because open source involves a community of developers always working to improve the code, open source actually encourages more rapid innovation and improvement than does a closed, proprietary system. And while it&#39;s taken several years to make this case, open source has seen widespread adoption in enterprise. Companies no longer balk (as much) at the thought of using tools like Apache, Linux, or Mozilla Firefox, all open source software. And while enterprise application remains much of the focus of open source development and of the OSCON conference, the event introduced a health care track, pointing to the myriad of ways in which open source technologies, along with open data, can enhance health care delivery.</p>
<p>	Health care was once something delivered by one family doctor, with our records stored in her or his memory and file cabinet. Now we are less likely to have such an intimate relationship with a single physician. There are new ways now we think about our health and track our well-being and a variety of ways in which we interact with the medical profession. </p>
<p>	As <a href="http://radar.oreilly.com/2010/07/wrap-up-of-the-health-care-it.html" rel="nofollow">Andy Oram</a> writes in his summary of the conference, the health care industry is slow to change for two reasons. First, there are numerous regulations, certifications, and control mechanisms in place that make innovation challenging. Health care workers as well as the software systems they utilized operate under stringent privacy and technical controls. Second, the actors themselves are, Oram argues, an independent bunch. Over 80% of medical practices in this country are operated by one or two physicians.</p>
<p>	Open source solutions are being applied to a number of areas in the health care industry, notably electronic records management. Presenters at OSCON included representatives from government agencies, corporations, and universities, including <a href="http://www.google.com" rel="nofollow">Google</a>, <a href="http://www.oemr.org/" rel="nofollow">OpenEMR</a>, and <a href="http://www4.va.gov/vista_monograph/" rel="nofollow">VistA</a> (the Veterans Administration&#39;s health data management system). And although there will be an uphill battle for widespread adoption and implementation, as Oram points out, proprietary software companies have yet to make sizable inroads themselves. Questions of standards, budget, training -- on top of an industry slow to accept the changes -- make the development of health care technologies challenging. But the need to provide better data and, simply, better health care is paramount. And the argument from OSCON is that open source development can help engage a community to work towards solutions.</p>

Inside the hospital, which cardio specialist is closest to the 4th floor? Outside the hospital - if an on-call physician is racing to get to a patient&#39;s side, how far away are they and when can they be expected to arrive?</p>
<p>
		From emergency to non-emergency to everyday preventative health care, location tracking technologies could make a big impact on our health and well-being in the future. While two million consumers use <a href="http://Foursquare.com">Foursquare</a> today to find the best nearby coffee shops and bars, what if in the future they used it to locate the best pediatricians, emergency clinics, or even restaurants that catered to their unique health needs? Some intersection between location and health care has already begun, but what we&#39;ve seen so far is likely only the beginning.</p>

	I showed my dental hygienist who else was checked-in there on Foursquare last time I went to the dentist&#39;s office and she was quite taken aback. But we consumers willingly shared our presence at and feedback about that medical facility. That&#39;s only the beginning of what&#39;s likely to happen.</p>
<p>
	If health insurance agencies track your location and charge you more for insurance, you&#39;re probably not going to appreciate that. There are some possible upsides to the intersection of location and health care, however.</p>

<p style="text-align: justify;">
	Nuestro sistema sanitario p&uacute;blico, pese a haber alcanzado un nivel alto de calidad, comienza a dar muestras de agotamiento; y eso no solo es debido a la escasez de recursos, sino tambi&eacute;n a la aparici&oacute;n de algunas tendencias que le inyectan presi&oacute;n. La primera es la conversi&oacute;n del ciudadano en consumidor: puesto que la Administraci&oacute;n se hace cargo de todo, las puertas del sistema hierven m&aacute;s de lo que seria necesario. Otra tendencia, esta epidemiol&oacute;gica, es el aumento de personas con una o varias enfermedades cr&oacute;nicas para quienes el modelo sanitario no es lo bastante eficaz, debido a que para lograr una mejora en su calidad de vida ser&iacute;a precisa su implicaci&oacute;n, adem&aacute;s de apoyo social y atenci&oacute;n continuada ajustada.</p>

	Como tercer elemento preocupante para el buen funcionamiento del sistema hay que destacar el incremento de ancianos en situaci&oacute;n de fragilidad. No hace tantos a&ntilde;os la gente de la tercera edad acababa sus d&iacute;as en casa, con muy poco apoyo m&eacute;dico. Ahora todo ha cambiado y parece justo que los ancianos gocen de la atenci&oacute;n sanitaria moderna, pero de ninguna manera la respuesta del sistema tendr&iacute;a que ser el amontonamiento de estas personas en boxes de urgencias. El &uacute;ltimo par&aacute;metro que quiero remarcar es el coste de la sanidad en relaci&oacute;n con el resultado obtenido en t&eacute;rminos de calidad de vida. En los hospitales se llevan a cabo acciones terap&eacute;uticas extraordinariamente caras con fines paliativos, y todos nos preguntamos cu&aacute;l es el l&iacute;mite del ensa&ntilde;amiento terap&eacute;utico. Pese a que el gasto econ&oacute;mico sanitario del acompa&ntilde;amiento del final de la vida es una exageraci&oacute;n, no parece que la sociedad est&eacute; por abordar este tema.</p>
<p style="text-align: justify;">
	Los sistemas sanitarios p&uacute;blicos (los que se pagan mediante impuestos, no p&oacute;lizas, y son universales) se encuentran con dificultades para abordar los retos y las propuestas de copago empiezan a surgir, ni que sea para poner orden a las puertas del sistema. Pero el tema de fondo no es el copago, sino c&oacute;mo implicar a los ciudadanos en su salud. Tomemos como ejemplo algunas mutuas privadas que establecen planes personalizados de salud con objetivos, que si los mutualistas los alcanzan, tienen reducciones. Este creo que es un camino que los sistemas p&uacute;blicos tendr&iacute;an que abordar. Pese a que no disponen de p&oacute;lizas, los gobiernos tienen, en cambio, formas de premiar o punir fiscalmente a los ciudadanos. S&eacute; que esta es una propuesta innovadora (a&uacute;n no probada en ninguna parte), pero se tiene que admitir que si se aborda la cuesti&oacute;n de la implicaci&oacute;n de cada ciudadano en su propio plano de salud, o en su plano terap&eacute;utico si lo requiere, con instrumentos m&aacute;s potentes (econ&oacute;micos por supuesto) que los bienintencionados consejos m&eacute;dicos, ser&iacute;a una manera resolutiva de preservar la accesibilidad realmente justificada al sistema sanitario p&uacute;blico.</p>

This is the English version of the interview. Matey was trained in law and new technologies and worked previously for the French Overseas and Sports Ministries. He can be described as an internet fan since his teen years.</p>

	<em>You are responsible for the French eHealth portal since January 2010. Please describe your activities</em></p>
<p>
		I have two main responsibilities, as a project manager and as an editor.&nbsp; As project manager, I coordinated the definition of the&nbsp; site map, our subjects, positioning, services, specifications. This was of course a team project and not a one-man activity.</p>
<p>
		As a medium, we have to be constantly looking at what&#39;s happening around us. However, as a public organization, we want to feature interesting initiatives and not just provide news. We&#39;ll be looking at our first results this fall and see what interest and expectations we have generated.</p>
<p>
	<em>What are the portal&#39;s goals ?</em><br />
	&nbsp;</p>
<p>
		France&#39;s public sector is striving to consolidate the development of health information systems in France. This portal&#39;s goals are to raise awareness of eHealth and to promote &quot;ASIP Sant&eacute;&quot;, the public agency piloting national efforts to exchange health data. Users of the site are most likely specialists of the subject, although it&#39;s open to all. Texts are written in easy-to-understand terms.</p>

						The national version of FAOSTAT,&nbsp;<a href="http://www.fao.org/statistics/countrystat/" target="_new"><strong>CountrySTAT</strong></a>, is being developed and implemented in a number of target countries, primarily in sub-saharan Africa. It will offer a two-way data exchange facility between countries and FAO as well as a facility to store data at the national and sub-national levels.</p>

	<a href="http://www.oscon.com/oscon2010/public/schedule/speaker/86701/?cmp=il-radar-conf-oscon10-dal-molin">Joseph Dal Molin</a>, one of founders of the <a href="http://www.worldvista.org/">WorldVistA</a> project and a speaker in the health care track at the upcoming <a href="http://www.oscon.com/oscon2010/public/schedule/topic/Health/?cmp=il-radar-conf-oscon10-dal-molin">OSCON convention</a>, recently spoke with me about the Department of Veterans Affairs&#39; historic <a href="http://www.ehealth.va.gov/">VistA</a> system and its expansion as an open source effort.</p>

	El pasado lunes tuve una vez m&aacute;s el placer de viajar a Barcelona para visitar a la gente de <a href="http://www.forumclinic.org/sobreforumclinic-es" onclick="javascript:pageTracker._trackPageview('/outgoing/www.forumclinic.org/sobreforumclinic-es');" target="_blank">Forumclinic</a> , una de las iniciativas m&aacute;s interesantes a nivel mundial en salud 2.0.</p>
<p>
	Es, seg&uacute;n sus propias palabras: &ldquo;un programa interactivo para pacientes destinado a que aumenten su grado de autonom&iacute;a con respecto a su salud, utilizando las oportunidades que brindan las nuevas tecnolog&iacute;as.&rdquo;, e impulsado por el <a href="http://www.hospitalclinic.org/" onclick="javascript:pageTracker._trackPageview('/outgoing/www.hospitalclinic.org/');" target="_blank">Hospital Clinic</a> de Barcelona y la <a href="http://www.fbbva.es/TLFU/tlfu/esp/home/index.jsp" onclick="javascript:pageTracker._trackPageview('/outgoing/www.fbbva.es/TLFU/tlfu/esp/home/index.jsp');" target="_blank">Fundaci&oacute;n BBVA</a>.</p>
<p>
	Entre otras muchas y excelentes iniciativas, han desarrollado unos foros donde pacientes de diversas enfermedades (Depresi&oacute;n, artrosis, diabetes, esquizofrenia, c&aacute;ncer de mama&hellip;) pueden compartir sus experiencias y sociabilizar, con la particularidad de que son supervisados por m&eacute;dicos especialistas en estas dolencias.</p>
<p>
	Durante mi visita, invitado por <a href="http://www.forumclinic.org/autores/imma-grau" onclick="javascript:pageTracker._trackPageview('/outgoing/www.forumclinic.org/autores/imma-grau');" target="_blank">Imma Grau</a>, una de las impulsoras de este proyecto, y probablemente la mayor experta de nuestro pa&iacute;s en salud 2.0 , tuve la oportunidad de charlar con estos m&eacute;dicos sobre el rol del Community Manager.</p>
<p>
	Para ellos, la experiencia de moderar un foro online era un desaf&iacute;o apasionante pero tambi&eacute;n complicado algunas veces. Si bien tienen a&ntilde;os de experiencia a la hora de tratar con enfermos, la din&aacute;mica de una comunidad online a veces les confund&iacute;a: &ldquo;&iquest;c&oacute;mo tratar con un troll. &iquest;Es conveniente que el moderador responda una por una a todas las dudas del foro, o es mejor que sea la propia comunidad quien se apoye unos en otros?. &iquest;Qu&eacute; hacer cu&aacute;ndo se cuelga en el foro informaci&oacute;n incorrecta?. &iquest;Hasta donde debemos ser estrictos en la aplicaci&oacute;n de las normas del foro?&rdquo;.</p>
